Digital Finger Printing

Current CPR Card

Valid Florida Drivers License

Valid Florida Auto Insurance

Valid Auto Tag/Registration

Proof of Alzheimer's and Related Dementia training

Proof of Assistance with Self-Administered Medication

Proof of HIV/Aids training

C.N.A. Certification

Valid C.N.A. License
